Perimenopause Fatigue: Patterns, Differentials, and When to Seek Care
Fatigue during the menopause transition may overlap with sleep disruption, bleeding and iron loss, thyroid disease, mood symptoms, medicines, infection or another condition. A pattern-based assessment is safer than a one-cause label.

What to record for an appointment
- Onset, duration, time of day and effect on work, exercise and daily tasks.
- Cycle and bleeding pattern, sleep schedule, night symptoms and snoring or gasping.
- Recent illness, pain, medicines, supplements, food intake and substance use.
- Mood, stress and any new neurologic, cardiopulmonary or bleeding symptom.
Urgent and crisis boundaries
Call emergency services for life-threatening symptoms. In the United States, call or text 988 for suicidal crisis or emotional distress; call 911 in a life-threatening situation.4 Severe breathlessness, chest pain, fainting, new weakness or confusion, or heavy uncontrolled bleeding also requires prompt emergency assessment.
